IVF success rates offer a general view of how treatment has worked for groups of patients. Your own chances depend on factors such as age, egg quality, sperm health, diagnosis, and treatment history.
Not every success rate measures the same stage of treatment. Some figures look at each IVF cycle started, while others focus on egg retrieval, embryo transfer, pregnancy, or live birth. Rates may also differ depending on whether treatment used fresh embryos, frozen embryos, donor eggs, or a patient’s own eggs.
This is why one percentage may look different from another. Your reproductive endocrinologist will review your testing, medical history, and treatment needs to discuss the factors that may affect your own IVF outcome.
How Does Age Affect IVF Success Factors?
Age is one of the strongest IVF success factors when a patient uses her own eggs. Both the number of available eggs and their genetic health change over time.
Ovarian reserve refers to the estimated number of eggs remaining in the ovaries. Hormone testing and an ultrasound follicle count give your fertility specialist an idea of how your ovaries may respond to fertility medications. These tests do not measure egg quality directly or predict the exact number of mature eggs an IVF procedure will produce.
Egg quality and IVF outcomes are closely connected because an egg needs the correct number of chromosomes to fertilize, develop into an embryo, implant, and continue through pregnancy. Chromosomal abnormalities become more common as egg age increases, which may lower implantation rates and raise the chance of pregnancy loss when using your own eggs.
IVF age success rates generally decrease as the age of the egg increases. In national treatment data, live birth rates from embryo transfers using patients’ own eggs ranged from 44.2% among younger patients to 12.3% among the oldest age groups. These figures describe broad groups and do not determine what will happen in your treatment.
Patients of the same age can still have very different outcomes. Ovarian reserve, sperm health, fertility diagnosis, embryo development, and previous pregnancy history all give your fertility specialist more information to plan treatment around your needs.
How Do Egg and Embryo Quality Affect The Success Of IVF?
The number of follicles seen during ovarian stimulation is not the same as the number of embryos that will be available for transfer.
Some follicles do not contain an egg. Some retrieved eggs are immature, and some mature eggs do not fertilize. Other embryos stop developing during embryo culture. This gradual reduction is a normal part of the IVF process.
Embryologists assess embryo quality by reviewing how each embryo grows and appears in the laboratory. Higher-quality embryos generally have a greater chance of implantation, but embryo grading cannot confirm whether an embryo has the correct number of chromosomes.
Preimplantation genetic testing (PGT) may give the treatment team more information before transfer. PGT for Aneuploidy (PGT-A) screens embryos for missing or extra chromosomes. PGT for Monogenic Disorders (PGT-M) looks for a specific inherited genetic condition when testing identifies a known risk.
PGT does not change embryo quality or increase the number of embryos available. It gives your reproductive endocrinologist more information when reviewing which embryo may be appropriate for transfer. The recommendation depends on factors such as age, embryo number, genetic history, previous pregnancy loss, and prior treatment results.
How Does a Fertility Diagnosis Affect IVF Success Factors?
IVF does not address every fertility issue in the same way. The diagnosis affects the stimulation protocol, expected egg response, fertilization method, and embryo transfer plan.
Conditions that may influence fertility treatment success include:
- Diminished ovarian reserve
- Polycystic ovary syndrome (PCOS), now referred to as Polyendocrine Metabolic Ovarian Syndrome (PMOS)
- Blocked or damaged fallopian tubes
- Fibroids or other uterine abnormalities
- Endometriosis
- Recurrent pregnancy loss
- Male infertility
- Unexplained infertility
- Known genetic disorders
Different fertility diagnoses affect different parts of IVF treatment. How Does Sperm Quality Affect IVF?
Sperm quality affects fertilization and early embryo development. A semen analysis measures sperm concentration, movement, and shape, although it does not measure every part of sperm function.
Low sperm count or poor movement may reduce fertilization through conventional IVF. In some cases, the laboratory uses Intracytoplasmic Sperm Injection (ICSI), which involves placing one sperm directly into a mature egg. ICSI is usually recommended when testing identifies male infertility, when a previous cycle had poor fertilization, or when the treatment plan includes certain forms of embryo testing.
Smoking, testosterone use, some medications, illness, alcohol consumption, and frequent exposure to high heat may also affect sperm production. Your treatment team may recommend further testing or lifestyle changes before the IVF cycle begins.
How Does Uterine Health Affect Embryo Transfer?
A healthy embryo also needs a uterine environment that supports implantation. Before embryo transfer, the fertility specialist assesses the uterine cavity and prepares the uterine lining.
Testing may identify:
- Fibroids or polyps that extend into the uterine cavity
- Scar tissue
- Congenital uterine abnormalities
- Fluid from a damaged fallopian tube
- A uterine lining that does not develop as expected
Endometrial thickness is one part of embryo transfer planning, but your fertility specialist will also review the appearance of the lining, hormone levels, uterine findings, medication timing, and embryo development. These details help determine when the uterus is ready for transfer and whether a fresh or frozen embryo transfer fits the cycle.
A fresh embryo transfer takes place soon after egg retrieval. A frozen embryo transfer takes place during a later cycle after the embryo has been stored. Your physician may recommend a frozen transfer when preimplantation genetic testing is planned, hormone levels need more time to settle, or the uterine lining would benefit from a different preparation.
The transfer plan also includes how many embryos to place in the uterus. For many patients, single embryo transfer offers a strong chance of pregnancy while lowering the risk of twins or a higher-order multiple pregnancy. As single embryo transfers became more common, twin births after assisted reproductive technology fell from 8.3% of embryo transfer cycles in 2012 to 2.1% in 2021.
Your physician will base the final recommendation on age, embryo quality, genetic testing, uterine findings, and previous treatment history.
Which Lifestyle Factors Affect IVF Outcomes?
Lifestyle changes cannot reverse age-related changes in egg quality, repair blocked fallopian tubes, or correct a genetic condition. They still affect general health, sperm production, medication response, pregnancy risk, and the safety of treatment.
Your fertility specialist may review:
- Smoking, vaping, and recreational drug use
- Alcohol consumption
- Weight and nutrition
- Exercise and sleep
- Prescription medications and supplements
- Chronic health conditions
- Heat or workplace exposures that may affect sperm
Smoking is associated with poorer fertility outcomes in both women and men. Patients should also discuss alcohol use with their physician because recommendations may change during ovarian stimulation, embryo transfer, and pregnancy.
Weight requires an individualized discussion. Being significantly underweight or carrying excess weight may affect ovulation, fertility medication dosing, egg retrieval, anesthesia, and pregnancy health, but a healthy weight will not look the same for every patient.
A balanced diet, regular exercise, and good management of existing health conditions support general reproductive health. No food, supplements, or eating plans guarantee successful IVF treatment or change the quality of an existing embryo. Instead, these factors become part of the broader treatment plan your fertility specialist develops around your health, diagnosis, and response to treatment.
Why Does an Individualized IVF Treatment Plan Matter?
An IVF cycle usually includes ovarian stimulation, monitoring, egg retrieval, fertilization, embryo culture, and embryo transfer or freezing. The details within each stage should reflect the patient’s diagnosis and treatment history.
